Major/Minor In-Game Choices

Episode 1

Reported Nathan - Ratted out Nathan Prescott to the Principal, telling Wells that Nathan was waving a gun around the girl's bathroom.
Comforted Victoria - Was nice to Victoria Chase after dumping paint on her, causing Victoria to become somewhat less abrasive than usual.
Intervened to help Kate - Stepped in to defend Kate when the school security guard was questioning her, comforting Kate and showing her she's not alone.
Took the Blame for Chloe's Pot - Hid in the closet when Chloe's stepfather came storming up to her room, but quickly came out and took the blame for the blunt on Chloe's desk, so that Chloe could escape David's wrath. This puts more of David's suspicion on her, but Chloe warms up much easier to her now.

Episode 2

Told Kate to Go to the Police - When Kate what she should do about a viral video of herself drunkenly (and unknowingly under the influence of drugs) kissing people at a party, Max says to bring it to the police.
Answered Kate's Phone Call - Chose to talk to Kate rather than Chloe, helping her calm down and listening to her when she was panicking. This helps much with the penultimate scene in episode 2.
Tried to Shoot Frank - When Frank pulled a knife on Chloe in the Junkyard, Max tries to shoot him to help Chloe. Thankfully the gun was empty!
Saved Kate's Life - After seeing Kate attempting suicide by jumping from the dormitory roof and rewinding (and STOPPING) time to get to the roof before she could do it again, Max managed to talk Kate down and get her to safety.
Blamed Nathan - When taken to the principal's office and questioned on who might have pushed Kate to suicide, Max laid the blame on Nathan drugging her at a party, leading to his suspension.

Episode 3

Stole the Money - Max and Chloe found an envelope containing $5,000 in the principal's desk. Chloe knows this can help her even things out with Frank, so Max allows her to take it.
Kissed Chloe: - The day after the late night pool shenanigans, Chloe jokingly dares Max to kiss her. Max does so.
Sides with Chloe - At breakfast later, Chloe and David get into a fight, where Chloe calls David out on his spying on several students and even setting up cameras in their house. Max supports Chloe in the argument, causing David to be kicked out of the house.

Episode 4

Accepted Chloe's Request - In the alternate future Max found herself in, where Chloe wound up completely paralyzed after a car accident, Chloe begged Max to help end her life, not wanting to keep living like that and be only a burden to her parents. After much heartache, Max accepted.
Frank Debate - Back in the right timeline, Max and Chloe realize they need Frank's list of contacts to decode the notes they swiped off of him earlier, to try and connect the dots between Kate's drugging, Rachel's disappearance, and Nathan Prescott. Thanks to the money stolen earlier, and the fact that they didn't hurt Frank's dog when stealing the notes in an earlier episode, they were able to resolve the confrontation peacefully.
Did not warn Victoria - Even with the knowledge that Victoria would be the next victim of the Dark Room, Max did not warn Victoria about this. This actually turns out to be a good choice, as Victoria won't accidentally tip the true mastermind off that something is up.

Episode 5

Sacrifice Chloe - In the end, Max and Chloe find out the real truth behind the catastrophic storm set to happen at the end of the week; Max using her powers to save Chloe in the beginning of the game unintentionally triggers several natural phenomenons, including the deadly hurricane that tears Arcadia Bay apart. This gives her two choices: let the hurricane destroy Arcadia Bay so that she can complete her goal of saving Chloe, or travel back to the beginning, and staying out of the conflict between Chloe and Nathan Prescott that ends with him accidentally shooting Chloe.

Extremely reluctantly, and only after much convincing from Chloe (and one final kiss), Max chooses to save Arcadia Bay.
